Chapter III - Iorveth's Path

You start out on a mountain path. Follow Iorveth into town. The path will be shorter if you imported a game where you sided with the Order. Either way, you'll get a cutscene where two soldiers tell you where Phillipa is, and Iorveth leaves your side. There are two ways to do get to her:
A) Get arrested by any of the guards in town. The group nearby will do. While breaking out, there will be a Quick-Time Event that's easy to miss. After Radovid leaves, Shilard will enter your cell. Be ready to start left-clicking like mad.
B) Sneak in through the Sewers. It's a long way around, avoiding guards as you go. Along the way, you'll probably have to complete "A Score to Settle." Continue along the outer wall, climbing and descending as necessary. You'll eventually come to the center of town, where you'll see a tower. Behind it is a door to go through. (If you find the door on the tower, you're on the opposite side of where you need to be.) Once in the sewers, when you come to what looks like a dead end, look for ledges to climb up or down.

However you rescue Phillipa, she'll give you a choice: help her with "The Spellbreaker" or do "Where is Triss Merigold?" The two quests are mutually exclusive. Helping Triss has fairly straightforward consequences. Helping Phillipa leads to lifting the spell that she has over Saskia, and you'll learn later that Letho will have rescued Triss for you.

Once done, it's time to finish all the side quests. "Bring it On: Loc Muine" is just southeast of the tower. As you approach the area south of the tower, "The Secrets of Loc Muine" will start. If you go back in the sewers (behind the tower), look for a chest. Inside are some diagrams which will trigger this Chapter's "Mystic River." The Notice Board starts "The Gargoyle Contract." Near the outer wall, along the northeast side, close to where you split up from Iorveth, you'll find the first four Dice players for "Poker Face: Loc Muinne." "A Score to Settle" is also in the same spot.

Once you've done everything and you're ready to finish the game, approach the Amphitheater and talk to either Iorveth or Triss, depending on whether you did "The Spellbreaker" or "Where is Triss Merigold?" They'll ask you to distract some guards, and then a series of cutscenes start. When you regain control of Geralt, you'll need to ascend a tower to find Sile. Your choice is whether or not to let her live. After that, fight a dragon. At the end of that fight you'll have a choice of whether or not to let her live. This leads into the Epilogue.
